Dear Parent/Legal Guardian:

We are pleased to welcome you to the Indian Hill Exempted Village School District. In order to enroll your custodial child in the District, please review and complete all applicable fields within this electronic form. This electronic enrollment form contains:

  • • General student and related contact info
  • • Education history
  • • Emergency medical information
  • • Residency information
  • • Annual parent notifications and consents

Once you have submitted the form, you will be directed to additional forms and documents required: birth certificate, proof of residency, medical/dental forms, and other required documentation. Please gather your materials, print the additional forms, and have them completed by the appropriate providers.

After your electronic information is submitted, please contact Tari DePoy, Central Registrar, at 513-272-4516 to schedule a student enrollment meeting. Please bring any additional materials and forms with you to that meeting.

If you are registering for kindergarten please fill out the PDF forms found at the end of the registration process and contact Katie Mileham at the Primary School at 513-272-4763 to arrange a convenient time for drop-off.

No student will be enrolled until all required forms are completed and supporting documents are supplied to the District. No student shall be discriminated against on the basis of race, religion, national origin, financial status, or disability.
